Founded in 1951, The Joint Commission is dedicated to continuously improving the safety and quality of the nation's health care through voluntary accreditation. An independent, not-for-profit organization, The Joint Commission is the nation's oldest and largest standards-setting and accrediting body in health care.
The Joint Commission evaluates and accredits nearly 15,000 health care organizations and programs in the United States, including more than 8,000 hospitals and home care organizations, and more than 6,800 other health care organizations that provide long term care, assisted living, behavioral health care, laboratory and ambulatory care services. The Joint Commission also accredits health plans, integrated delivery networks, and other managed care entities. In addition, The Joint Commission provides certification of disease-specific care programs, primary stroke centers, and health care staffing services.

Our dedicated stroke rehabilitation team focuses on providing a higher level of care: access to daily medical management, a multi-disciplinary approach, specialty rehabilitation nursing, revolutionary technology and expert therapy.
Additionally, HealthSouth Rehabilitation Hospital of York's Stroke School of Recovery is offered as a vital component to our comprehensive inpatient stroke program and is dedicated to providing patients and families with invaluable education on stroke related topics. We believe that education is fundamental to a stroke patient's recovery, as well as their future wellness.
Joint Commission Disease-Specific Certification in Brain Injury Rehabilitation
HealthSouth Rehabilitation Hospital of York is proud to offer a comprehensive Brain Injury program that achieved Joint Commission Disease-Specific certification in February 2010. Superior clinical expertise, state of the art rehabilitative technology, and a comprehensive continuum of care are just a few qualities of the program. HealthSouth York has a dedicated Brain Injury Rehabilitation Team, which is led by physiatrist Dr. Daniel DeFalcis, and includes ten Certified Brain Injury Specialists (CBIS) certified through the Academy of Certified Brain Injury Specialists, and twelve certified rehabilitation nurses (CRRN).
HealthSouth York’s comprehensive continuum of care begins with inpatient rehabilitation that includes sensory stimulation management. Outpatient services include customized outpatient therapy, a specialized Neuro Treatment Program with a focus on community re-integration, a Post-Concussive Program, and a Neuro Work Hardening Program.
HealthSouth York’s Brain Injury School of Recovery is an individualized patient and family education program and is offered as a vital component of HealthSouth York’s comprehensive program.
Joint Commission Disease-Specific Certification in Diabetes Mellitus
HealthSouth Rehabilitation Hospital of York is proud to offer a comprehensive Diabetes Mellitus program that achieved Joint Commission Disease-Specific certification in September 2011. Our dedicated team provides an interdisciplinary approach to the management of diabetes, including comprehensive assessment, health screens for risk factors, education in self-management and medical recommendations. Superior clinical expertise is provided by physicians, physical therapists, occupational therapists, registered dietitians, specialized rehabilitation nurses, a certified diabetic educator and registered pharmacists.
HealthSouth York's comprehensive continuum of care begins with inpatient rehabilitation that includes interdisciplinary assessments, foot screens, medication management and discharge planning. Our team is dedicated to providing patients and family members with the skills to manage diabetes. Education is provided in both group classes and individually, and includes topics such as: an overview of diabetes, hyper and hypoglycemia, complications of diabetes, nutrition education, foot care, sick time management, medication management and exercise.
